Assessment for Social Care

What is an Assessment?

An assessment helps us to decide what kind of help may be available. Every person's situation is different and wherever possible we help people to keep their independence so they can continue to live in their own homes.

We will fully involve you in your assessment and will ask what you can do for yourself and what type of help and support you need. You can have someone with you when we discuss your needs.

Carers are entitled to an assessment in their own right to discuss what help may be needed with caring. For more infomation, please visit the Carers UK website

We would like to provide services for everyone who asks for support but the demand is greater than the money available for services.

How do I apply for an Assessment?

To apply for an Assessment, you can:

What happens next?

During your assessment:

  • We will consider your views and wishes.
  • You can have a friend, relative or someone else to help you or speak for you, if you want.
  • We will arrange for an interpreter if you do not speak English or if you use sign language.
  • We will consider your carer's views.
  • The people involved in your assessment will work together effectively.
  • We will give you information about the services that are available and any charges involved.

After your assessment, we will tell you:

  • Which services you will get, who will provide them and where and when they will be provided.
  • The name of the person who is your social worker or is responsible for managing your care.

How do we decide if you are eligible for services?

All councils have to use government guidance to work out whether a person is eligible for services. The guidance is 'Fair Access to Care Services' (FACS) and is to ensure that services go to people who are most in need using Eligibility Criteria

Not everyone will qualify for services but if there are other sources of help we will tell you about them.

If you qualify for services we will give you a written Care Plan showing the services to be provided, who will do what, when they will do it and whom to contact. We will review your services regularly to make sure that they are continuing to meet your needs.

Can I organise my own equipment or services?

Yes. If you would prefer to organise your own services or equipment you may want to take the option of Direct Payments. This means that instead of the Council organising the services/ equipment that your assessment shows you need, you can receive the money and arrange your own services/ equipment. This gives you more choice and control.
