Early Education Support Grant Application

This funding is only available to support children attending an early education funded place. Your application must include a Trafford provision map. You can also include individual plans or reports from other professionals to support your application. You can upload these documents in the supporting information section of this form.

Please use this section to provide the supporting documentation for your application.

You are required to provide a completed Provision Map describing the support you will put in place to meet the child's needs and the outcomes you hope to achieve. You can download a provision map template at www.trafford.gov.uk/eysendfunding

Please provide a copy of the child's latest Wellcomm screening.

If you have identified the child's main area of Special Education Need as Social Emotional and Mental Health (SEMH), please provide a copy of their Ages and Stages Questionnaire, Social and Emotional (ASQ3:SE).

You may also wish to provide additional supporting information. This could include individual plans, 2 year progress check or reports from other professionals.

This application must be made with the full knowledge and consent of the child's parent/carer. Applications can not be discussed at a panel meeting if the parent/carer consent form has not been received.

There are two ways to do this. The parent can complete an online consent form or you can print a form for them to sign and upload it below. Both forms are available at www.trafford.gov.uk/eysendfunding
