Short breaks application form

  • Weekend and Holiday Clubs

    Short breaks are activities for disabled children and those with additional needs aged 5 (school year 1) to 18 (school year 13).

    If you have parental responsibility for the child, please complete the application form to apply. Once submitted, you will be contacted directly by the short breaks provider. A separate application will need to be completed for each child. If you are applying to access a club during holiday time only, please ensure you only apply for SENSE or Sport Works.

    The SENSE service is for children and young people with a written diagnosis for ASC. If your child is not eligible for the SENSE offer you may still apply for the Sport Works short breaks.

  • Child's gender
  • What is your child’s primary disability? If you are applying for SENSE then please tell us if your child has any additional disabilities/needs alongside their autism diagnosis.
  • Complex Needs Service
  • Do you have a named social worker or social care reviewing officer?
  • Do you have a personal budget / direct payment?
  • If your child is known to the Complex Needs team, an amount will be deducted from your Personal Budget if your child does go along to these activities.
  • Charges
  • There is no charge for these sessions, however the provider may charge for individual activities. Providers will inform you of any charges before sessions take place.
