Short breaks application form

  • Weekend and Holiday Clubs

Short breaks are activities for disabled children and young people and those with additional needs aged 5 (school year 1) to 18 (school year 13), who are unable to access mainstream services and who are not in receipt of a short break support package.

If you have parental responsibility for the child, complete an application form via the short breaks portal to apply. Once submitted, you will be contacted directly by your chosen short breaks provider. A separate application will need to be completed for each child.

To find out more about short breaks and activities visit Trafford Directory Short Breaks.
