Personal Alcohol Licence Change of Address

Before you start...
This form is required if a personal licence holder changes their residential address, even if they move out of the borough. To submit this form you will be required to provide these documents: A new passport style photograph, copy of your driving licence or passport as ID and a Utility Bill or bank statement dated within 3 months. A fee of £10.50 will be required, however, a payment link will be sent to you via email when the Licensing Officer has checked your application.
