This form is for early education and childcare providers intending to deliver early education funded places. Completing this form confirms you agree to the terms of the Provider Agreement.

The provider will then be included in Trafford's Directory of Early Education Providers.

It is the provider's responsibility to ensure the form has been completed correctly. Forms that do not have the required information will not be processed.

This declaration must be completed, signed and authorised by:

The Ofsted Registered Person, or
A Director of the business who can be verified with Companies House (for limited companies), or
A trustee of the organisation who can be verified with the Charity Commission for England and Wales (for registered charities)

Childminders registered with a childminder agency should select option 3 and confirm the name of the agency.

All other childminders should select option 1.

1) A setting registered on the Ofsted Early Years Register under section 40 of the Childcare Act 2006

2) A provider that is exempted from delivering the Early Years Foundation Stage EYFS learning and development requirements under section 46 1 or 2 of the Childcare Act 2006

3) A childminder registered with a Childminder Agency under section 37 of the Childcare Act 2006 .
