Declaration form for schools

This form is for schools intending to deliver early education funded places. Completing this form confirms you agree to the terms of the Provider Agreement.

The school will then be included in Trafford's Directory of Early Education Providers.

It is the school’s responsibility to ensure the form has been completed correctly. Forms that do not have the required information will not be processed.

This declaration must be completed, signed and authorised by the Headteacher or Executive Headteacher.
