DoLS applications from care homes and hospitals
If a person is aged 18 or over, lacks mental capacity, and may need to be deprived of their liberty in a care home or hospital, the provider (the managing authority) must apply for authorisation.
How to make a referral
1.Go to the GOV.UK DoLS resources page to download the correct form:
- Form 1 – standard and urgent (initial) requests
- Form 2 – renewal or further authorisation
- Form 10 – request a review
2. Submit your completed form through the Trafford DoLS Portal.
Relevant Person’s Representative (RPR)
If you are cared for under a DoLS authorisation, you will have a Relevant Person’s Representative (RPR).
Your RPR will:
- make sure your rights are protected
- help you share your wishes and feelings
- visit you regularly to see how you are doing
- speak up if your care arrangements are not in your best interests
- ask for a review or challenge the DoLS authorisation if needed
Challenging an unlawful Deprivation of Liberty
If you believe a friend or family member is being deprived of their liberty without the correct authorisation, you should raise your concern directly with the hospital or care home.
If no action is taken, write to the Supervisory Body; in most cases, this will be Trafford Council. You can speak to the DoLS team by calling 0161 912 2036 or emailing dols@trafford.gov.uk.
