If you have additional needs, a disability, or a long-term health condition, you might need extra support as you move from school or children’s services into adult life.
The Council and its partners can help you plan for this change, known as the transition to adulthood.
Get support
If you are 18+ and require support, complete the online referral form. You can also call adult social care on 0161 912 5199 or email adultsocialcare@trafford.gov.uk.
If you are under 18, you will need to be referred by children’s social care. If you are already in touch, you can speak to children's social care about preparing for adulthood by calling 0161 912 5125.
