Assessments during transition

If you are referred by children’s social care and are likely to need adult social care after age 18, you will be offered a social care assessment. This looks at your needs, goals, and what support will help you live well as an adult. Find out more.
You will need to complete a financial assessment to decide how much you need to contribute towards your care. You can find more information on paying for care and support as an adult.
Whoever is supporting you can also have their own Carer’s Assessment to make sure they get the right support too.
You have the right to:
- Be involved in all decisions about your care
- Have information in a format that works for you
- Have an advocate if you find it hard to express your views
If funding is provided through health rather than adult social care, your support may be coordinated by health professionals, such as a community nurse.
