Continuing your recovery at home
After you leave hospital, you may be offered support to help you recover and live independently at home.
Referrals can be made by a healthcare professional or social care worker.
Care at home service
If you’ve had a short illness or an operation, you may only need some short-term support to help you recover. Our Care at Home service provides reablement support in your own home for up to six weeks (with a target of three weeks).
The aim is to help you regain confidence and independence so you can live safely at home. You can read more about care after hospital on the NHS website.
How to access this service
If you meet the criteria, you will be referred from the hospital before your discharge. Following this, a social care worker will be in touch within 14 days to carry out an assessment to understand your ongoing care and support needs.
The service is free for up to six weeks; however, most people only need up to three weeks of support.
After two weeks, if you are assessed that you need social care services or support, we will talk to you about the type of services which may suit you and how much they will cost.
Stabilise and Make Safe (SAMS)
If you need extra support after an illness, an accident or a hospital stay, then you could be referred to the SAMS Service by the hospital before your discharge.
The SAMS service offers short-term intensive support to help you in your own home. Support is free for up to six weeks; however, most people only need up to three weeks of support. The service is based on your needs, so the type of support and number of visits will reduce as you no longer need them.
A social care worker will visit you to agree on realistic goals to help you regain confidence and independence. After three weeks, if you are assessed that you need social care services or support, we will talk to you about the type of services which may suit you and how much they will cost.
The service is available seven days a week during working hours. The service usually starts between Monday and Friday.
Trafford Community Response, Discharge to Assess Service
Trafford community response is a community service that supports you to leave hospital sooner. The service carries out assessments you need at home rather than waiting for it to be completed in hospital. Find out more about Discharge to Assess.
Ongoing care
If you continue to need care or support after these services, we’ll help you access the right ongoing support.
You may be referred to Adult Social Care for further assistance. If long-term care is needed, a financial assessment will be completed to determine how much you may need to contribute towards the cost of your support.
