Have you ever been convicted of an offence involving the breach of a condition regulating or prohibiting a performance of hypnotism?

I understand that by submitting this application, I consent to Trafford Council passing on the data within this application and it's supporting documents to any authority or person that will assist it's determination of this application, or as required by law?
