Chaperone reference

You have been asked to provide a reference for an applicant for the role of Children’s Chaperone in Entertainment. The applicant will have unsupervised access to children from the very young (pre-school) up to and including age 16s. Read the information page before completing this reference. If you feel that you are unable to act as referee, indicate your reasons as to why you are unable to provide a reference.

We cannot accept references from people who are solely a friend or a relative or partner of the applicant. Bear in mind that you should ensure the reference is accurate and does not contain any untrue statement or omission as the reference may be revealed to the person concerned, if requested.
