Agreement for Providers of Early Years Funded Places 2024-27

This form is for Childminder Agencies who have childminders in Trafford intending to deliver funded places.

Completing this form confirms you agree to the terms of the Provider Agreement 2024-27 v1 and countersign with the individual childminder named below. A copy of the provider agreement is available on our website.

To receive funding providers must read the agreement, then complete and submit this form. Ensure the email address your provide is correct as you will receive a copy of your submission by email. You must keep a copy of the agreement and this form for your records. Ensure this form has been completed correctly, we are unable to accept forms that do not have all the required information.

This agreement must be completed and signed by the Ofsted Registered Person or for limited companies a director of the business who can be verified with Companies House.
