Agreement for Providers of Early Years Funded Places 2024-27 schools

This form is for schools intending to deliver early education places. Completing this form confirms you agree to the terms of the Provider Agreement 2024-27 v1. A copy of the provider agreement is available on our website.

To receive funding providers must read the agreement, then complete and submit this form. Ensure the email address your provide is correct as you will receive a copy of your submission by email. You must keep a copy of the agreement and this form for your records. Ensure this form has been completed correctly, we are unable to accept forms that do not have all the required information.

This agreement must be completed by the Head or Executive Head.
