Agreement for Providers of Early Years Funded Places 2024-27

This form is for childcare providers intending to deliver early education funded places. Completing this form confirms you agree to the terms of the Provider Agreement 2024-27 v1. A copy of the provider agreement is available on our website.

To receive funding providers must read the agreement, then complete and submit this form. Ensure the email address your provide is correct as you will receive a copy of your submission by email. You must keep a copy of the agreement and this form for your records. Ensure this form has been completed correctly, we are unable to accept forms that do not have all the required information.

This agreement must be completed and signed by the Ofsted Registered Person or for limited companies a director of the business who can be verified with Companies House.

Confirm your registration status

Childminders registered with a Childminder Agency should select option 3 and confirm the name of the agency. All other childminders should select option 1.

To be included on the Directory of Early Education Providers this agreement must be authorised by the Ofsted Registered Person or for limited companies a Director of the business who can be verified with Companies House. Should the Manager, Ofsted Registered Person or Director change, it is your responsibility to notify the Early Years Service.
