Documents you need
You must upload colour copies of the following documents:
- a basic criminal disclosure certificate issued within one month from the Disclosure and Barring Service
- disclosure of convictions and declaration
- your personal licence qualification certificate
- proof of address dated within 3 months i.e utility bill or bank statement
- two passport style photographs, one signed by a recognised professional such as a doctor, solicitor, teacher, magistrate or minister. To sign your photo, a recognised professional must complete a form of endorsement of photograph
- proof of right to work in the UK, such as
- a British or EU passport, which can be expired
- a full birth or adoption certificate issued in the UK
- a current Immigration Status Document from the Home Office
- a right to work share code
You may be asked to bring the original documents to an in person appointment.

- Select Licensing from the categories.
- Select Apply.
- Select Personal - New.
- Make your payment and keep the receipt reference.
Make a note of your receipt number, as you’ll need it when you complete the application form.
