Change your name

You must tell the licensing authority if your name changes, for example after marriage or by deed poll.

If you are a Designated Premises Supervisor, you must also tell the premises licence holder.

Documents you need

• marriage certificate or deed poll certificate
• new passport style photograph

How to pay

1. Go to our payments page
2. Select Licensing from All categories
3. Select Apply
4. Choose Personal Variation, Notice, Amendment or Copies
5. Make your payment and note your receipt reference

Make a note of your receipt number, as you’ll need it when you complete the application form.
