The paperwork you will need for a cremation

Once you have chosen cremation, you will need to submit the relevant paperwork to us to begin the process.
The relevant forms you will need to complete are:
- Application for cremation of the body of a person who has died
- Application for cremation of a stillborn baby
- Application for cremation of body parts
If you are having a service, you will also need to complete the cremation particulars form.
Alongside the forms above, you will need to submit the 'Green Form’, from the Register Office or The Coroner’s Certificate.
